Neighborhoods That Encourage Walking Reduce Obesity and Diabetes

{鼓励更多步行的邻里有助于减少肥胖病和糖尿病}

In an earlier post, Why Walking is Good, Walking is Good because anyone can do it at any age and any fitness level. Plus it is good for your heart, your head and your wallet.  Here is why Walking is extremely good for post-transplant patients – 2-3 weeks after my surgery, I started with walking and am still doing it.  An exercise recommended by most Renal doctors.  I intend to continue my Walking for the rest of my life.

My latest post demonstrates the importance of neighborhood planning and walking contributing to the reduction of Obesity and Diabetes.
In accordance to the article from American Diabetes Association, some important points are as follow:

·           Base on a 10-year period study in Canada, more "walkability" spaces within the neighborhoods would lead to lower risk of developing diabetes.

·           Diabetes fell 7% but rise 6% for those who live in least walkable neighborhoods.
·           Overweight and obesity fell by 9% for those live in most walkable neighborhoods.

Hypoglycaemia {Update#1} (低血糖)

In this update, I wish to share my recent experience of hypoglycaemia and what is the appropriate ways to treat it under different circumstances.

In one of my meals, I took slice fish and vegetable soup WITHOUT any carbohydrates items, such as brown rice and wholemeal bread, to go along.  At exactly two hours after my meal, my glucose level was 3.9mmol/L.  Please read on how to treat Hypoglycemia.

According to the Dietitian, I have to include a portion of carbohydrates item in all my meals to avoid Hypoglycaemia.  Without any carbohydrates item, the system/insulin will use up the any carbohydrates item as store in my body.

What is Hypoglycaemia?

It is a medical term for Low Blood Sugar.  The principal problems arise from an inadequate supply of glucose to the brain.  The most common forms of hypoglycemia occur as a complication of treatment of diabetes mellitus with insulin.

What is the acceptable glucose level?

In local term, the acceptable glucose is above 4.0 mmol/L. Symptoms of hypoglycmia will develop when the glucose falls below 4 mmol/L

Symptoms of hypoglycemia that I have experienced:
·      Sweating
·      Shaking or trembling
·      Hunger
·      Mood changes such as sad or angry
·      Confusion
·      Anxiety

Other reports mentioned that if blood sugar drops very low, the person may not be able to respond, becomes unconscious or has a seizure.

How to treat hypoglycaemia under following circumstances:
·      If you experience hypoglycaemia symptom with a blood sugar that is 4.0mmol/L and above, treat it with a small carbohydrates snack.

Treatments
Take a small carbohydrates snack
Take 15-20g of rapid-acting carbohydrates, e.g.:
Ø Take 3-4 teaspoons of sugar / glucose power
Ø 4-5 glucose tablets
Ø 150-200ml sweetened soft drinks / juice

Take 30g of rapid-acting carbohydrates, e.g.:
Ø Take 6 teaspoons of sugar / glucose power
Ø 8 glucose tablets
Ø 300ml sweetened soft drinks / juice

Further actions
No further actions are required
Re-test after 15 minutes – is the blood sugar >4.0mmol/L?
Follow-up actions
Not Applicable
YES – Depend on when is the next as a starchy carbohydrate may be necessary
NO!!
Ø Repeat the above step until blood sugar >4.0mmol/L
Ø Seek treatment immediately if blood sugar <4.0mmol/L for 2 cycles
Follow up treatment POST hypoglycaemia
Within 1 hour
NO additional carbohydrates snack.  Go ahead to have meal.
More than 1 hour
Take long acting carbohydrates, e.g.: biscuits or bread.
Personally, I take one slice of wholemeal bread.
Do note that this is only a general guideline.  Discuss with your doctor and adjust according to requirements.

As a diabetic, I always carry my Nano, needed strips and lancets wherever I go.

1.     What is Hypoglycemia?
It is a medical term for Low Blood Sugar.  The principal problems arise from an inadequate supply of glucose to the brain.  The most common forms of hypoglycemia occur as a complication of treatment of diabetes mellitus with insulin.

2.     What is the acceptable glucose level?
In local term, the acceptable glucose is above 4.0 mmol/L. Symptoms of hypoglycemia will develop when the glucose falls below 4 mmol/L

3.     Symptoms of hypoglycemia that I have experienced:

·      Hunger
·      Mood changes such as sad or angry

Other reports mentioned that if blood glucose drops very low, the person may not be able to respond, becomes unconscious or has a seizure.

4.     How to treat hypoglycemia under this circumstances:
·      At home and in the office – Once hypoglycemia is experienced, take 3 spoons of sugar mixed with water to drink.  Alternately, take normal coke as it has high sugar contents.  15 minutes later, check the glucose level to ensure it is above 4.0 mmol/L.  You may need to repeat the same process again if it is still below 4.0 mmol/L.
·      Prior to an exercise – Either take cereals prior to the exercise or bring along a chocolate bar to your workout

As a diabetic, I always carry my Nano, needed strips and lancets wherever I go.

Two and half month after my transplant, a lab test was taken on 14th July 2014.  After reviewing the results as well as other conditions, my Renal Doctor decided to reduce the evening dosage of my Cyclosporine (Neoral®).

I did not do anything unusual in the manner of taking my drugs as compare to my earlier post “Post-Transplant Medications” (我没有做什么异常以服我的药方):
1.     I always take my drugs on time (我总是准时服我的)
2.     I take my drugs with HL milk for better absorption and for my daily calcium intake (我使HL牛奶服药达更好的吸收和完部分每日钙的需)

Do take note that renal doctor DID NOT adjust the dosage on the basis of test results alone.  There are other indicators that he will review before making the final and crucial decision. (注意à 肾脏医生没有根据单独测试结果调整剂量。他在做出最后和关键的决定前将回顾的其他显示)


Next item that is up for review will be my dosage of Mycophenolate mofetil (Cellcept®), another drug that I am taking to help preventing or treating rejection.

Skin Cancer (皮肤癌)

Organ transplant patients are at a higher risk — up to a 100-fold higher — for developing skin cancer compared to the general population.
This higher risk is caused by immunosuppressive medications, which are essential to transplant patients to prevent graft rejection and optimize graft survival. Because these medications suppress the immune system that fights off infection and prevents the development of cancer, transplant recipients are at elevated risk for infection and certain cancers.

An article, Warning Signs of Skin Cancer, by MOSS GREENE, may not be transplant related but provides information in identifying skin cancer.
